Conflict of Interest Questionnaires

Vendors doing business with a governmental entity are required by State law to submit a Conflict of Interest Questionnaire. This questionnaire must be filed with the records administrator of the local government not later than the seventh business day after the date the person becomes aware of the facts that require the statement to be filed.
